From I-5 Heading South:

Take the 217/Kruse Way exit (292).  Turn left and cross over I-5 onto Kruse Way. Follow Kruse Way to Boones Ferry. Turn right on SW Boones Ferry Rd. Go one mile, then turn right into Olson Memorial Clinic. First building on left.

From I-5 Heading North:

Take the Tualatin/Lake Oswego exit (290). Turn right onto Boones Ferry Rd. Go one mile, cross train tracks, up a rise, then turn left at Olson Memorial Clinic. First Building on the left.
